Patient presented into clinic with pressure injury to an area of all scar tissue. How to stage pressure injuries in 100% scar tissue.

Hi Carla, thanks for the question. I have not seen a revised or separate staging system for pressure ulcers in relationship to scar tissue. Nor have I seen this with regard to areas previous covered with skin grafts or flaps. The depths of the individual skin and tissue layers may be different in both scar tissue and grafts/flaps, but the ulcer staging would be the same as the outlined in the NPIAP (National Pressure Injury Advisory Panel).
